YOUR FIRST VISIT

Choosing a new dentist is a big decision and we appreciate the confidence you have placed in us. We hope that our website and the conversations you've had with our office coordinator, while making your appointment, have made you more familiar with our office and with us. A new patient dental exam is very in-depth and adults should plan to be in our office for up to two hours; it is important that you do not bring small children to your appointment with you. Teens thirteen and older will be with us up to an hour and a half and children twelve and younger up to an hour. Please be sure to arrive 15 minutes early so paperwork can be finalized before your appointment.

Before your arrive at the office for your visit, please be sure to do the following things:

• Fill out or print all forms, which are online for easy access.
• Bring your insurance information.
• Please request a Release Form. This is an authorization for disclosure of information. (We will gladly contact your previous dental care provider.)
